Renewal/Reinstatement

Renewing your license is easy with our new eLicensing portal.

 

Please read the statement regarding disclosure of criminal history prior to completing your application.

Complete the steps below to submit your renewal application:

1 Scan your current, unexpired CPR Card.  
2 Scan your current BOC Card (if you are still certified).

Update Fingerprints

  • If you have obtained a new fingerprint clearance card since your most recent license was issued, scan the new card.  
  • If you do not have a fingerprint clearance card and it has been more than five years since you provided fingerprints, go to your local law enforcement office and have your fingerprints rolled onto FBI form FD-258.
  • If neither of the above apply to you, you do not have to provide and fingerprint documents.
 
4 If your citizenship status has changed, complete the Statement of Citizenship form. (Arizona Revised Statutes § 41-1080 requires, in general, that a person applying for a license must submit documentation to the license agency that satisfactorily demonstrates the applicant’s presence in the United States is authorized under federal law.) 5 If your citizenship status has changed, scan the document that proves your citizenship status. List of Accepted Documents
6 Complete your online renewal application.
