Pursuant to A.R.S. § 41-1080, if you earned less than 200% of the Federal Poverty Guidelines, you may qualify for a one time waiver of the Board’s application fee of $300.00. If you think you may qualify for this fee waiver, please:
- Contact the Arizona Department of Revenue (“ADOR”) at 602.716.6072 and request a waiver; and
- Once you obtain a fee waiver from ADOR, immediately forward it to the Board in order to administratively complete your application.
Please note: The Board has no input regarding how the ADOR determines if you qualify for the fee waiver. Also, the ADOR’s fee waiver decision-making process may significantly delay the processing of your application with the Board. If the Board does not receive an official ADOR fee waiver, on your behalf, within 120 days of the date your application is processed and a deficiency letter is mailed to you, your application will be closed pursuant to the Board’s licensing timeframes (A.A.C. R4-49-205). If your application is closed, you may reapply for a license after obtaining the ADOR fee waiver.
